How to register / temporary residents

Please print off and complete GMS1 this registration form and bring to the surgery with the additional information as below.  Alternatively you can collect a copy from our Patient Care Advisors.

You will also need to fill in a health questionnaire click here and bring in some form of photographic identification and proof of your current postal address, ie recent utility bill (less than 3 months old).

Overseas patients

If you are visiting the area from overseas and wish to see a doctor at the practice, the following policy applies:

  • All visitors from overseas are entitled to free emergency care at the practice.
  • In addition, visitors from countries in the European Economic Area (and Switzerland) are entitled to free care at the practice for pre-existing conditions. These visitors will however have to pay for any prescriptions.
  • Visitors from countries outside the European Economic Area (or Switzerland) will have to pay for a consultation at the practice as well as any prescriptions (if the treatment is not deemed to be an emergency by the GP).

If you find it hard to read our letters or if you need someone to support you at appointments, please let us know.

  • We want to know if you need information in braille, large print or easy read.

 

  • We want to know if you need a British Sign Language interpreter or advocate.

 

  • We want to know if we can support you to lip-read or use a hearing aid or communication tool.

 

Please tell the receptionist if you need information in a different format or communication support.
